Referral Process

Referrals are received primarily from the States of Indiana and Ohio, although we have admitted children from as far away as Alaska, Florida, and New Mexico. Wernle Children's Home, Inc. accepts children from the Department of Child Services, Probation Departments, Indiana Department of Education, Indiana Department of Correction and Juvenile Courts.
A client accepted at Wernle Children's Home, Inc. must be male and between 6 and 21 years of age.
Documentation requested to determine if a child is appropriate for placement is the child's social history, psychological/psychiatric evaluation and intelligence testing, original birth certificate and social security card. Wernle Children's Home, Inc. accepts children with a full scale I.Q. of 60 or higher. However, each client is looked at on an individual basis and test scores do not always reflect a client's potential abilities. Emotional and mental health issues, as well as learning disabilities may depress scores.
If a client is accepted, the Admissions Coordinator will send the placing agency a notice of acceptance and request the following documents for completion and /or review at the time of admission:
- Signed State or Wernle Placement Agreement
- Court order naming Wernle Children's Home, Inc. as placement
- Copy of Social Security Card
- Prefer original birth certificate (original is needed for a Bureau of Motor Vehicles Identification Card) our Transition Living Unit residents are required to have this form of identification to obtain employment.
- Interstate Compact (this is for out-of-state placements only)
- Signed agency complaint form
- Department of Child Services case plan
- Insurance card and/or Medicaid card (Insurance/Medicaid cards are used for all medical, dental and optical needs).
Special note: All Medicaid cards must be Traditional and not Managed Care.- Individual Education Plan (IEP), grades/report cards
- Dental, physical and vision exams, immunizations and TB testing
- Clothing list
- A prescription or two week supply of medication (if applicable) is needed when new residents are admitted.
At the time of admittance, all items should be brought with the child by the placing agency. Transportation of the client to our facility is also offered upon request. If Wernle transports the child, all documentation should be faxed or sent prior to admittance. Clothes and personal items may be picked up with the client. Children may not bring expensive items to Wernle. Wernle takes no responsibility for expensive personal items such as televisions, musical instruments, cameras or computer stations, nor will Wernle be held responsible for replacement of such items should they be lost, damaged or stolen.
